Rates
Rates for joining the plan can be calculated per member by using the “Division Rates for [Year]” and “Division Rates Calculation Sheet” spreadsheets available on the Forms page of the Health Insurance section.
Enroll as an employer in the plan
Any church that is a part of the BGC Canada association of churches may enroll in the plan. A church wishing to enroll as an employer in the pension plan will need to its financial administrator or treasurer fill out the Employer Application Form for Insurance.
On the application form, please note:
That an effective date on the top right hand side of the first page should be included and “New Application” checked off.
Life Insurance/Accidental Death & Dismemberment (AD&D), Long Term Disability (LTD), and Extended Health Care are benefits that are mandatory to every employer application.
* Note on Life Insurance & AD&D: With Life Insurance & AD&D – Most churches choose to provide “2x annual earnings”. If this option is chosen, it is important to note that when an employee’s salary changes any time after enrolment the plan administrator must be notified of the salary change in order for the employee’s coverage to accurately reflect his or her new salary.
Dental Care, Short Term Disability, Semi-Private Hospital, and Vision Care are benefits that are optional for the employer to provide.
* Note on Short Term Disability: Churches in BGCC usually do not offer coverage as the Canadian government generally provides adequate short term disability for employees in most people’s opinion.
Regarding the Employer Premium Contribution, it is required that all churches associated with the Baptist General Conference of Canada pay 100% of the overall monthly premium.
Once the financial administrator or church treasurer has completed the forms, the original copy should be mailed to the BGCC Office. A record will be made in the office, and the application will be forwarded to the CCCC for approval.
Pre-authorized payment
As an option to churches enrolled in the Insurance Plan, churches may sign up for automatic monthly withdrawals as a preferred method of payment. If your church is enrolling for the first time as a new employer, please mail the original signed form along with the signed Employer Application Form to BGC Canada. If your church is already enrolled as an employer in the plan, you may fax the completed form to the number at the bottom of the page.
